Patients will be confirmed with an appointment time and an information packet will be sent out prior to their first appointment. Included in the information packet will be several sheets that will need to be completed prior to their appointment time. You may also download our New Patient Information Packet Forms: Welcome New Patient to JIS, Patient History Form, Demographic Information Form, Billing Information & Consents, JIS Privacy Notice & Acknowledgment, JIS-Starpath-NASH map, and Mansfield Clinic Map (if applicable).  All files require Adobe Acrobat Reader. Completing these ahead of time will assist us in making your appointment as convenient as possible.

• Previous x-rays or MRI results
• A list of current medications
• A physician referral (if required by your insurance)
  
  

• Because the physicians may need to examine your joint closely you may want to wear loose fitting clothing or shorts if your being seen for a knee problem.
